Fully upgraded Pirate Gear (male armor and Matt's default armor) can randomly summon it with 1/6 or 16.(6)% chance. The Narwhal Tamer medal is awarded when the summon appears for the first time. Giant Squid can also summon Narwhal, which won't trigger the medal.

Found in a treasure chest at Crystal Caverns, specifically on the second block puzzle screen, inside the room with a battle that appears as an Ice Golem, requiring to solve the puzzle to enter it.
